Dr. Jordan Leasure, B.A., D.C., C.C.W.P., FMACP
Dr. Jordan Leasure is the Founding Physician of North Shore Pro-Active Health. She was born and raised in Libertyville, graduated from Lake Forest College, majoring in Chemistry and Business, and became a licensed Chiropractor in 2006.
She went on to receive her Certified Chiropractic Wellness Practitioner certificate in 2010, and is currently pursuing her Diplomate in Nutrition through the Institute of Functional Medicine and Functional Medicine University. Being a former collegiate athlete, her focus is on functional medicine and sports injuries.

Dr. Jordan Leasure is the Founding Physician of North Shore Pro-Active Health. She was born and raised in Libertyville, graduated from Lake Forest College majoring in Chemistry and Business and became a licensed Chiropractor in Illinois in 2006. Dr. Leasure received her Certified Chiropractic Wellness Practitioner certificate in 2010, and is currently pursuing her Diplomate in Nutrition through the Institute of Functional Medicine and Functional Medicine University. Being a former high school and collegiate athlete; her focus is functional medicine and sports injuries.
Dr. Leasure participates regularly in local organizations and is a past president in her local BNI, a Past Vice-Chairman for the GLMV Chamber of Commerce, she sat on the Board of Directors for Mainstreet Libertyville and is currently the Northern District President for the Board of Directors for the Illinois Chiropractic Society.
She maintains her sanity by riding her Peloton & meditating. Her 8-year-old son, Parker, picks the meditation they listen to before bed! Dr. Jordan and her husband spend most of their day running after their 3-year-old, Atlas on his two wheeler!
 
Dr. Jade (Leasure) Dellinger, B.A., D.C., C.C.W.P., FMACP
Also a lifelong Libertyville resident, Dr. Jade studied nutrition at the University of Illinois before joining Dr. Jordan at Palmer College of Chiropractic in 2005. She became a licensed Chiropractor in Illinois in 2010.
Dr. Jade is dedicated to family care focusing on educating parents on the importance of chiropractic for their children and caring for pregnant women throughout and after their pregnancies. Dr. Jade is Webster Certified, became certified by the Academy Council of Chiropractic Pediatrics in 2018, and a B!RTHFIT Professional in 2020. Her preferred methods of treatment include diversified adjusting, kinesotaping, craniosacral therapy, and other rehab modalities.

As of 2020, she is a B!RTHFIT Professional with a goal of helping women not only feel mentally, physically and spiritually prepared for birth, but to achieve the birth THEY desire. Her preferred methods of treatment include diversified adjusting, kinesotaping, craniosacral therapy and other rehab modalities.
In 2025 she completed a pediatric functional medicine certification through the Institute of Functional Medicine so that she can better guide her patients who are looking for alternatives to western medicine for their children.
She is currently working on a certification called Developing Minds, where the mission is to prevent DIS-EASE in the unborn child, to promote optimal neurodevelopment in the newborn and to create a life of optimal health for all!
When she isn’t working she enjoys traveling, being outdoors, spending time with her husband and being a boy mom to 6-year-old Jamison and 2.5 year old Grady.

As a busy mom of two, she understands the value of staying energized, balanced, and proactive with your health.
Originally from Burlington, WI, Kortnee now calls Grayslake, IL home, where she lives with her husband Steven and their two adorable kids— Connor (7) and Rosie (2). Whether she’s helping patients on their wellness journey or chasing giggles at home, Kortnee brings warmth, energy, and a genuine passion for living well to everything she does.
 
Oliva Rivas, Rehab Assistant - Elite Trainer
Originally from Lake County, Olivia is a certified Elite Trainer through ISSA with a passion for functional rehab and strength training. Her approach is rooted in lived experience, helping clients build confidence, resilience, and whole-body wellness from the inside out.
After overcoming personal health challenges and serving in the U.S. Army, Olivia brings both empathy and discipline to her work- ensuring every client feels supported and capable.

Our Rehab Assistant, Olivia, grew up in the Lake County area. After high school, she became obese after developing an eating disorder. Years of bullying, no confidence and trying many things that strengthened her eating disorder drove her to change. She told myself it was all or nothing, there had to be a way. She read books and studied on fitness and decided to try bodybuilding by herself. She quickly fell in love with it. It was something that brought her discipline, strength and so much more. She never gave up or looked back and ended up transforming her whole life and found her passion.
She then studied through ISSA to get her Elite Trainer certification to work with average people all the way up to athletes/bodybuilders. When she joined the Army as a very fit individual, she ended up getting hurt to the point of being disabled. She has used many methods of rehab to help keep her going. Thus finding her way into the rehab sphere of learning. She never take clients through anything that she hasn’t or wouldn’t do herself.
Her goal is to help everyone achieve a healthy lifestyle that is not just about how you look on the outside but how you feel on the inside and how you function. She also have a big passion to help women see that they are capable of many things. Outside of work she enjoys the outdoors, the gym everyday, yoga, meditation, violin and creative art.
 
Cindy Poland, Rehab Assistant - E-RYT, Certified Barre Instructor
Born in Chicago and raised in Glenview, Cindy brings over two decades of experience in movement and wellness to her role. She is an experienced yoga and barre instructor with advanced training in prenatal yoga and myofascial release.
Her personal experience with sports injuries and raising three competitive dancers gives her a deep appreciation for integrative care. Cindy is passionate about helping patients recover, rebalance, and reconnect through breath, movement, and fascia-focused rehab.

Our Rehab Assistant, Cindy was born in Chicago, raised in Glenview and spent the last 20+ years raising her own children in the house her grandfather built in Libertyville. She received her BS in Accounting from the University of Illinois and began her career as a litigation consultant for a Chicago Big 6 accounting firm. After her first child was born, she kept her feet in the accounting world doing bookkeeping for her mom’s business and serving as Treasurer for a local chapter of her sorority. Yoga has been a cornerstone of Cindy’s life since studying for the CPA exam post college. She is an E-RYT, certified Barre instructor and teaches Yoga, Meditation, and Barre locally. She has extra certification and training in prenatal yoga and myofascial release. She enjoys helping expecting moms prepare to welcome their little ones and working with athletes of all ages. Cindy has deep appreciation for the benefits of chiropractic care and physical therapy after working through her own sports injuries and those of her three competitive dancing daughters. She has a passion for helping others with sports recovery, pain, and injury through rehab exercises, yoga, fascia release, stretching and breath work. Finding ways to weave movement into the busy lives of our patients is important to help them find better health and balance. Cindy loves spending free time with her husband, daughters, dog, traveling, reading, walking and getting outside.
 
Agata Foreman, LMT
Originally from Poland, Agata found her calling in the healing arts after moving to the U.S. at age 20. Her academic journey through ECC, Roosevelt, Harper, and MCC led her to First Institute’s Massage Therapy program, where she graduated with honors.
Agata is certified in Manual Lymphatic Drainage, Marma Point Therapy, Trigger Point, Cupping, and Prenatal/Postpartum care. Her work blends technical skill with intuitive touch, offering clients a deeply restorative experience rooted in compassion and care.

Agata was born in Poland and the US become her home when she was 20 years old. Since she can can remember, Agata was fascinated with healing arts.
Agata studied at ECC, Roosevelt University, Harper and MCC. Throughout her studies she discovered the power & effectiveness of Therapeutic touch and decided to pursue it and enrolled in the First Institute Massage Therapy. Program and graduated with honors.

Agata currently lives in Cary on beautiful Lake Killarney with her two children, two dogs and a cat.
 
Jill Pohlman, LMT
With over 12 years of experience, Jill’s massage practice is rooted in intuitive touch and client-led care. Her hybrid style draws from deep tissue, myofascial release, trigger point, prenatal, sports, and relaxation techniques.
Jill believes healing is a partnership, one that honors the body’s signals and the client’s voice. She avoids tools, preferring hands-on connection to guide each session toward balance, relief, and whole-person wellness.

Having over 12 years of massage experience, Jill's practice has evolved to a customized, hybrid style that can involve all of the skills she has acquired over the years. Her experience includes deep tissue, myofascial release, trigger point, pre and post natal massage, sports massage, and swedish/relaxation style massage.
She prefers to not use tools, but use her intuitive touch to communicate with issues going on within your tissues. Listening to what the client needs is crucial to healing and she believes in following the client’s lead for how they need the massage to be for them.
Healing mind, body, and soul is an important partnership that can be essential for a client with the right therapist, which she strives to be for each client.
 
Jorell Ellazar, Social Media Assistant
Jorell earned his master’s in Clinical Nutrition from Rush University and completed his dietetic internship at Harper College. He began his journey with North Shore Pro-Active Health in 2019, supporting functional medicine, marketing, and patient education.
With a background in foodservice and a passion for holistic wellness, Jorell helps clients build sustainable habits through nutrition and lifestyle support. Outside the clinic, he enjoys cooking, martial arts, and spending time with loved ones.

Jorell is a Rush University alum having earned his master’s degree in Clinical Nutrition in December 2021 and has also finished his dietetic internship from Harper College in 2025.
As an undergraduate student from Lake Forest College, he first worked for North Shore Pro-Active Health as a Functional Medicine intern in 2019 and then was employed as a part-time Chiropractic Assistant. Since then, he’s taken up various tasks to assist the company with its marketing tasks, social media presence, and functional medicine services.
Jorell hopes to take his experience in foodservice, education in dietetics, and passion for wellness to those seeking food and nutrition related help. With the help of North Shore Pro-Active Health he aims to help his team reinforce the importance of a healthy lifestyle for overall well-being. In his free time, Jorell enjoys cooking for his family, spending time with friends, weight-lifting, and practicing martial arts.
